Description The Opportunity

This is a unique and meaningful opportunity to work with Holocaust survivors and support their independence, well-being, and dignity as they age. As a bilingual HCS Care Manager, you will assess needs, coordinate resources and financial assistance, and connect clients with supportive services and community initiatives. Working with the HCS team, you will support a caseload across office, home, community, and remote settings while participating in supervision, case consultation, and program activities.

This opening may be filled at the HCS Care Manager II or Care Manager/Supervisor level based on qualifications and program needs. Both levels maintain a caseload; the Supervisor also guides Care Managers and provides oversight on complex cases. This full-time, salaried, hybrid role follows a Monday-Friday, 8:30 a.m.

-5:00 p.m. schedule, with some flexibility as appropriate. Pay is $55, for Care Manager II or $65,000-$75,000 for Care Manager/Supervisor. SEIU Local 73 membership is optional for Care Manager II; the Supervisor is non-union.

What You’ll Do
  • Maintains a client caseload, including conducting regular office-based, in-home, or remote assessments of Holocaust survivors’ physical, functional, emotional, financial, and environmental systems.
  • Determines eligibility for financial assistance and other supportive services.
  • Completes documentation in a timely manner.
  • Provides ongoing case management, resource linkage, and support services to clients as needed.
  • Participates in weekly individual supervision/case consultation and weekly team meetings.
  • Participates in program planning and implementation, including cultural and socialization programs such as “Café Europa,” Holocaust commemorations, and holiday and birthday celebrations.
  • Depending on professional interest, may provide counseling and/or co-facilitate survivor support groups or classes.
  • Based on professional interest, may lead or participate in professional presentations and community education; a s opportunities arise, may attend trainings and conferences locally or nationally.
  • Additional responsibilities at the supervisor level:
    • Provides formal supervision to assigned Care Managers, including weekly individual supervision and ongoing case consultation.
    • Reviews and approves client assessments and service determinations for quality, consistency, and compliance with program and funder requirements.
    • Guides staff in managing complex situations involving trauma history, cognitive decline, medical needs, financial insecurity, social isolation, and family dynamics.
