The LHL Medical Case Manager Team Leader provides leadership, supervision, and support to direct reports while also providing medical case management services to people living with HIV at Erie Family Health Centers.

• Team Leader Supervision:
  Provide 1-on-1 supervision, leadership, coaching, and support to direct reports. Complete new HIV intakes, manage the HIV pool in the EMR system, and assign cases/patients to appropriate team members. Manage staff schedules, time-off requests, staffing coverage, and assist with hiring, onboarding, and training. Monitor team workflows, performance metrics, and daily activities to promote efficiency and achievement of program goals. Conduct quality assurance of case management documentation and provide feedback, guidance, and support to direct reports.
  
  Communicate new initiatives, workflows, and program changes, provide regular updates to the LHL Program Manager, and assist with special projects and assigned meetings.
• Case Management and Program Support:
  Manage a reduced caseload of higher-need, high-complexity HIV+ clients and provide medical and non-medical case management services in accordance with agency policies and procedures. Coordinate medical treatments, treatment adherence counseling, benefits enrollment, resource referrals, and access to Emergency Financial Assistance (EFA) to support positive health outcomes.
