Position Summary

The Refugee Health Promotion Case Manager & Outreach Coordinator plays a dual role in supporting refugee communities through direct case management and community outreach. This position focuses on improving health outcomes by connecting individuals to services, promoting preventive health behaviors, and increasing program participation through culturally and linguistically appropriate engagement strategies.

Key Responsibilities Case Management & Client Support
  • Provide individualized case management services, including intake, assessment, service planning, referrals, and follow-up.
  • Enroll clients in monthly health-related workshops and wellness groups to support engagement and positive health behaviors.
  • Conduct strong and consistent follow-up with clients to encourage retention and active participation in health workshops, wellness groups, and one-on-one sessions.
  • Monitor client participation and track program data, ensuring accurate documentation of client progress and experiences.
  • Maintain timely and compliant case notes, records, and reports in alignment with program requirements.
  • Monitor client progress and adjust service plans as needed to support positive outcomes.
  • Advocate for clients to reduce barriers and promote equitable access to services.
Outreach & Community Engagement
  • Develop and implement targeted outreach strategies to engage refugee and immigrant communities.
  • Assist in promoting program services to potential participants through outreach activities and community engagement.
  • Ensure that all outreach materials and program delivery are culturally appropriate and linguistically accessible to maximize interest and participation.
  • Plan and coordinate health education workshops, group sessions, and community events.
  • Conduct presentations and outreach activities in collaboration with community partners.
  • Build and maintain strong relationships with community- and faith-based organizations, including mosques, cultural centers, foundations, associations, and school districts.
  • Collaborate with partners to identify and engage individuals who may benefit from program services.
  • Represent the organization at community events, resource fairs, and stakeholder meetings.
  • Coordinate outreach logistics, including event planning, scheduling, materials, and interpreter services.
  • Support participant recruitment, enrollment, and ongoing engagement and follow-up.
  • Track and document outreach activities, participation, and outcomes to measure program effectiveness.
  • Collaborate with internal teams to ensure smooth program operations and service coordination.
  • Contribute to program improvements by incorporating community feedback and best practices.
Qualifications Education & Experience
  • Bachelor’s degree in Public Health, Social Work, Communications, or a related field (preferred).
  • 2–4 years of experience in case management, outreach, or community-based programming.
  • Experience working with refugee, immigrant, or underserved populations strongly preferred.
  • Strong interpersonal and community engagement skills.
  • Demonstrated cultural competency and ability to work effectively with diverse populations.
  • Strong organizational and documentation skills.
  • Ability to manage multiple responsibilities across direct service and outreach functions.
  • Ability to work flexible hours, including evenings or weekends as needed.
Preferred Qualifications
  • Multilingual skills relevant to the populations served.
  • Experience with trauma-informed care and cross-cultural programming.
  • Familiarity with refugee resettlement systems and community-based services.
