Description
Outreach & Recruitment· Conduct community-based outreach to identify and recruit eligible IFRP participants.
· Develop and maintain relationships with community organizations, schools, workforce agencies, social service providers, and other referral partners.
· Attend community events, job fairs, resource fairs, orientations, workshops, and recruitment activities.
· Present program information to prospective participants and community partners.
· Distribute flyers, recruitment materials, and program information through approved outreach channels.
· Conduct phone, text, email, and in-person outreach to prospective participants.
· Maintain an active referral pipeline and follow up with individuals who express interest in the program.
· Conduct community informational's about public benefits available to immigrants in IL.
· Facilitate community workshops and serve as an interpreter when attending outreach events and workshops.
· Work in collaboration with fellow team members to secure support in attending the various outreach events.
· Prepare the marketing materials needed to be delivered at each event, ensuring that program flyers and marketing giveaways are available at all times.
· Collect the outreach sign-in logs at the conclusion of each event and deliver them to the Program Director for review and proper tracking of efforts in the system.
Participant Engagement & Intake· Explain program eligibility requirements, services, expectations, and available opportunities to prospective participants.
· Assist participants with completing applications, enrollment forms, and required documentation.
· Conduct initial screenings and collect information needed to determine program eligibility.
· Schedule and facilitate orientations and enrollment appointments.
· Follow up with participants who have incomplete documentation or have not completed enrollment requirements.
· Maintain consistent communication with participants throughout the enrollment process.
· Identify barriers that may prevent participation and connect individuals with appropriate resources.
Case Management Support· Work collaboratively with Case Managers to ensure a smooth transition from outreach and enrollment into program services.
· Make referrals to employment, education, training, transportation, housing, childcare, financial assistance, and other supportive services as appropriate.
· Monitor participant engagement and follow up with individuals who become disengaged.
· Document participant contacts, referrals, services, and outcomes accurately and timely.
· Assist with participant retention and follow-up activities.
· Organize internal and external workshops to educate the community on the importance to access income supports services, such as Medicare, Medicaid, All Kids, SNAP, and case assistance benefits through the Illinois Department of Human Service (IDHS) to eradicate the myth of “becoming a public charge” when an eligible person access these services.
Community Partnerships· Establish and maintain positive relationships with community stakeholders and referral partners.
· Develop referral networks that support program enrollment and participant success.
· Conduct presentations and informational sessions for partner organizations.
· Represent the organization professionally at community meetings and events.
· Identify new outreach opportunities and strategies to reach priority populations.
· Collect resources on recreational activities, housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health services
· Initiate, establish, and nurture a close working relationship with other community-based organizations, agencies, schools, and healthcare facilities to promote the income supports services offered under the Immigrant Families Resources Program (IFRP).
· Assists client to develop natural resources and contact social support networks.
Data, Documentation & Compliance· Enter outreach, referral, enrollment, and participant contact information into required databases and tracking systems.
