The Primary Care Physician (PCP) is a licensed/Board Certified/Board Eligible trained professional in internal or family medicine who provides direct patient care in ambulatory settings, with possible assignments in acute care, nursing homes, SNFs and home settings depending on the assignment. Responsibilities include geriatric assessment, medical history, physical exams, diagnosis and treatment, development of the plan of care, health education, referrals, case management referrals, follow-up and documentation according to Chen Med standards for quality, service, productivity and teamwork.
The PCP may participate in clinical rounds and documentation through progress notes and summaries.
The PCP demonstrates the ability to function independently and in collaboration with other health care professionals and will consult with managers and medical directors to ensure compliance with guidelines, participate in risk and quality management programs, clinical meetings and other required meetings. The PCP adheres to departmental goals/objectives, standards of performance, regulatory compliance, quality patient care, and policies and procedures.
EssentialJob Duties / Responsibilities
- Functions independently as a primary care practitioner as part of a patient care team.
- Independently assesses acute and non-acute clinical problems.
- Performs and documents physical assessments and patient histories, analyzes trends, and develops, documents and implements a patient management plan. Includes patient/family counseling and education.
- Plans patient care based on knowledge of the patient population and protocol, considering cultural background, level of understanding, personality and support systems. Serves as patient advocate.
- Patient management includes: writing admission/transfer/discharge orders; ordering/ interpreting labs and diagnostics; ordering medications and treatments; referring for consultations as indicated; documenting progress notes and summaries.
- Participates in patient care rounds and conferences; communicates management strategies to the care team.
- Collaborates with the multidisciplinary team to ensure patient care needs are met.
- Recognizes urgent situations and initiates life-saving procedures when necessary.
- Uses advanced communication to problem solve complex situations and improve processes and service.
- Collaborates to analyze and evaluate current systems to identify and implement new practice patterns.
- Participates in activities that enhance personal and professional growth and development.
- Initiates discharge arrangements and completes related paperwork.
- Works with physicians, nurses, PTs, social workers, family and caregivers to transition patients to appropriate levels of care.
- Advocacy & Education—ensures patient has an advocate for needed services and education.
- Introduces self to patient/family and explains PCP role.
- Facilitates patient/family conferences to review goals, optimize resources, provide education and identify needs.
- Facilitates collaboration to maximize informed decisions regarding goals of care, palliative care and hospice where appropriate.
- Utilization/Financial management—manages resource utilization and reimbursement for services.
- Facilitates discharge to appropriate level of care and uses preferred providers when additional services are required.
- Critically analyzes data and evidence for improving clinical practice.
- Integrates knowledge from the humanities and sciences.
- Translates research and other knowledge to improve practice processes and outcomes.
- Develops new practice approaches based on integrating research, theory, and practice knowledge.
