RN Case Manager

General Summary

Seeking a full‑time candidate to work as an RN Case Manager. Responsibilities include performing post‑discharge calls, case management evaluations, and acquiring services for discharge planning.

• Perform post‑discharge calls to ensure patients continue to do well, answering any questions or concerns.
• Assess new patients by gathering information, reviewing diagnoses, and analyzing medical test results.
• Develop complete plans of care to address patients’ needs, including evaluating those at risk of adverse events or re‑admission.
• Educate patients about their care plan and health care options, helping them make key decisions regarding health and post‑discharge options.
• Schedule appointments for doctors’ visits, therapies, and other medical services.
• Revise care plans based on patients’ progress and input from physicians and other providers.
• Update patient medical records after each contact.
• Offer emotional support for patients and their families.

• Registered Nurse
• Available to work part‑time
• Available to work some weekends
• Experience with Case Management (Preferred)
