Prime Healthcare Illinois Medical Group is seeking a dedicated and compassionate Nurse Practitioner or Physician Assistant (NP/PA) with experience to join our growing Primary Care team
. This is a rewarding opportunity to provide comprehensive, patient-centered care in a well-established outpatient setting.

Position Overview:

• Full-time position in an outpatient primary care clinic
• See patients for preventive care, chronic disease management, and acute visits
• Collaborate closely with a team of physicians, APPs, and clinical support staff
• Manage your own patient panel with opportunities for continuity of care
• 40 hour/week schedule with minimal call responsibility

• Licensed and certified as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Illinois
• Board-certified in Family Medicine, Adult-Gerontology, or related field
• Prior experience in primary care preferred
• Strong clinical assessment, communication, and documentation skills
• Commitment to providing high-quality, compassionate care
