Fetal Cardiology Nurse Coordinator

The Fetal Cardiology Nurse Coordinator leads the coordination of care for patients with fetal congenital heart disease, working across multidisciplinary teams to ensure seamless prenatal and postnatal planning. This role manages all aspects of prenatal care—including testing, counseling, care transfers, and procedure coordination—while advocating for patients and developing individualized care plans. The coordinator collaborates closely with physicians, nurses, referring providers, and hospital staff to ensure timely communication of clinical information and alignment on care and delivery plans.

Responsibilities
  • Maintain patient tracking systems.
  • Monitor fetal echocardiograms across multiple sites.
  • Support high‑risk deliveries.
  • Serve as the primary point of contact for patient and family communication.
  • Support continuous quality improvement, contribute to research initiatives, and foster a patient‑centered environment aligned with organizational goals.
  • Coordinate specific program activities involving clinical, education, and research.
  • Complete assessments, identify goals and objectives for performance improvements, and develop care plans.
  • Participate in the development, implementation, and ongoing assessment of policies, procedures, and guidelines that define the patient population.
  • Develop and manage clinical databases to track key performance indicators.
  • Establish and maintain rapport among a diverse group of health‑care providers and maintain effective working relationships.
  • Act as liaison between the division and community‑based programs.
  • Transition patients from inpatient to outpatient settings when appropriate.
  • Support patient education on clinical diagnosis and provide families with information on community resources.
  • Document and communicate all team, physician, and patient/family communication and concerns in the medical record.
  • Act as a resource to hospital personnel regarding care, coordination, and evaluation of patients in the division.
  • Participate in planning, implementing, and disseminating research and/or quality improvement projects.
  • Provide education to nurses, patients, families, residents, physicians, and ancillary health‑care workers.
  • Maintain current knowledge of patient care via professional societies, literature, and educational conferences.
  • Perform job functions with customer‑service focus, innovation, service excellence, and teamwork.

Knowledge,

Skills and Abilities
  • Bachelor of Science in Nursing from an accredited institution (required).
  • Master’s degree in nursing preferred.
  • Registered Professional Nurse licensure in the State of Illinois (required).
  • Minimum of three years of pediatric nursing experience (required).
  • Effective communication with patients/families, physicians, hospital staff, social agencies, school personnel, and community resources.
  • Excellent organizational skills and ability to manage multiple competing responsibilities.
  • Successful experience in new program development, coordination, and marketing.
  • High level of initiative and autonomy.
  • Proficiency in Word, Excel, Power

    BI, PowerPoint, and knowledge of Epic.
  • BLS certification (required).
Education

BSN in Nursing (required). Master’s degree in Nursing preferred.

Salary

$83,200.00‑$ per year.
