Job Details
- Must have BLS certification (American Heart Association or Red Cross).
- Must have valid Illinois nursing license (Nursys verification).
- CPI certification required.
- Must pass Prime Basic Pharmacology Exam with 80% or higher.
- Must pass Prime Specialty Exam with 80% or higher.
- Prime Education Verification required (diploma or transcripts accepted if third party verification unavailable).
- Driver's License, State , or Passport required.
- Prime OIG check required.
- Prime Drug Screening (minimum 10-panel test) within 60 days of start, including THC.
- Prime Color-blind test completed annually.
- Prime Agency Attestation required.
- Prime SAM/EPLS exclusion check required.
- Up-to-date resume required at offer acceptance.
- Two professional references required, one from a manager or direct supervisor.
- COVID-19 vaccine: 2 doses plus booster or declination completed.
- MMR:
Proof of 2 vaccines or positive titer showing immunity. - Annual Fit Test for respirator mask required prior to start.
- Hepatitis B immunization record, immune titer, or declination accepted.
- Influenza vaccine or declination required during flu season.
- TB test within one year of start date.
- Varicella vaccination or titer showing immunity.
- Tdap vaccination required; declinations not accepted in some states.
- Annual TB questionnaire required.
- Physical/Health statement annually, must state clear to work.
- Prime Core Competencies Exam with 80% or higher.
- Prime Skills Checklist required.
- National Criminal Background Check within one year of start date; some states require before assignment.
- Prime Badge Photo (JPEG) required.
- Prime Form I-9 (Employment eligibility verification) required.
- Signed Prime Job Description from within the year required.
- DCI/BVP certification for Workplace Violence Prevention required.
- Completion of Essentials Onboarding Packet and Onboarding Education Attestation required.
Job Requirements
- Dayshift: 7:00 AM to 7:00 PM.
- Must be available to work June 11 to June 12.
