Peterson Park Health Care Center provides post‑acute care and nursing services at its location on the Chicago Northwest Side. The facility specializes in post‑hospital rehabilitation, wound care, short‑term respite stays, and a dedicated Evergreen Unit for residents with dementia and Alzheimer’s disease. It is located at 6141 N. Pulaski Road, Chicago, IL.

We are looking for Licensed Practical Nurses to join our team at Peterson Park. Your work will ensure residents receive the medication and nursing care they need, conduct assessments, lead teams, and advocate for residents’ comfort and safety.

• An unencumbered State of Illinois Licensed Practical Nurse (LPN) license.
• Prior LPN experience preferred; willingness to invest in the right candidate.

As an LPN, you will serve as the backbone of the clinical department: ensuring residents receive prescribed medication, performing resident assessments, leading the team, advocating for residents, and maintaining safety and comfort.
