Wound Care Coordinator

We are seeking a compassionate and dependable Wound Care Director to join our team and play a key role in ensuring the health, comfort, and dignity of our residents. As a Wound Care Director, you will provide hands‑on care to residents, identify changes in condition, and evaluate care needs. You will also monitor overall condition and provide care in compliance with facility policies and procedures.

Responsibilities
  • Assess, plan, and implement individualized wound care treatment plans in accordance with established standards of care
  • Conduct wound inspections and assess wound status, documenting findings and progress
  • Develop and implement treatment plans, including wound cleansing, dressing changes, and other interventions as needed
  • Educate patients and family members on wound care and prevention strategies
  • Collaborate with interdisciplinary healthcare team to ensure comprehensive care
  • Participate in quality improvement initiatives to enhance wound care outcomes
  • Maintain accurate and confidential patient records
  • Provide hands‑on care to residents based on their Plan of Care
  • Identify changes in resident condition and evaluate care needs
  • Demonstrate strong assessment and documentation skills
  • Evaluate and participate in the development of the Resident Plan of Care
  • Monitor overall condition and provide care in compliance with facility policies and procedures
  • Provide primary skin care to residents under the medical direction of the attending physician or Director of Nursing
  • Identify, manage, and treat skin disorders, lesions, and decubitus ulcers
Requirements
  • Current and valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state
  • At least 2 years of clinical experience in a long-term care, rehab, or skilled nursing setting
  • Prior experience providing wound care in a post-acute or long-term care facility
