Job Type: Part-Time / PRN
What You’ll DoAs a dual-certified Nurse Practitioner, you will provide comprehensive, whole-person care across medical and psychiatric domains in skilled nursing and post-acute settings.
Post-Acute / Primary Care Responsibilities- Perform comprehensive physical exams and review medical histories
- Diagnose and manage acute and chronic medical conditions
- Order and interpret diagnostic testing (labs, imaging, etc.)
- Prescribe medications and develop individualized treatment plans
- Collaborate with physicians, nursing staff, and interdisciplinary teams
- Educate patients and families on disease management and prevention
- Document clinical encounters accurately in the EHR
- Conduct psychiatric evaluations, follow-ups, and consultations
- Assess patients for mental health conditions and behavioral concerns
- Develop and implement individualized treatment plans
- Evaluate need for higher levels of care, including hospitalization
- Provide medication management and monitor treatment adherence
- Deliver patient and family education related to psychiatric care
- Work closely with supervising physicians and care teams to optimize outcomes
- Maintain strict confidentiality and high standards of client-centered care

Required Qualifications- Master’s degree in Nursing (MSN) from an accredited institution
- Minimum of 2 years of clinical experience preferred
- PMHNP certification required for psychiatric scope
- DEA and applicable state prescribing registration (or eligibility)
- Ability to practice independently and manage diverse patient needs
- Strong communication, documentation, and collaboration skills
- Adult and geriatric populations
- Skilled nursing, post-acute, primary care, or psychiatric settings
- Experience with EHR systems and autonomous documentation
