About the Role
We’re seeking an Attending Nurse Practitioner to provide comprehensive, proactive medical management for residents across long-term care, ventilator units, behavioral health units, and subacute/post-acute rehab. You’ll round regularly, coordinate interdisciplinary care, and use decisive, evidence-based interventions to improve outcomes and reduce avoidable hospitalizations.
Key ResponsibilitiesProvide comprehensive medical management for patients in long-term care, ventilator, behavioral health, and subacute/post-acute settings.
Conduct regular/daily rounding in assigned facilities to ensure continuity of care, close monitoring, and timely follow-ups.
Aggressively manage chronic and complex conditions; respond promptly to changes in status and emergent situations using appropriate modalities (e.g., IV diuretics, antibiotics, fluids, TPN) and respiratory therapies.
Proactively prevent avoidable hospitalizations through early intervention, vigilant surveillance, and effective clinical decision-making.
Collaborate with facility nursing staff, rehabilitation teams, leadership, and the attending/collaborating physician to coordinate and optimize care plans.
Discuss and manage goals of care and palliative care options with patients and families; support shared decision-making.
Promote healthy aging and wellness through preventive care and proactive treatment plans.
Document thoroughly and promptly in the EMR; maintain high standards for clinical documentation.
Actively participate in interdisciplinary care meetings and care-planning processes.
Board certification as an Adult, Geriatric, or Family Nurse Practitioner (AGNP, GNP, or FNP).
Active, unrestricted NP license in the state of practice; national certification required.
Current BLS (BCLS) and ACLS certifications.
Prior experience in long-term care, subacute rehab, ventilator care, or chronic disease management preferred.
Strong clinical assessment and diagnostic skills; familiarity with ventilator care, IV therapy, and post-acute protocols preferred.
Excellent communication, documentation, and organizational skills; proficiency with EMR systems.
Demonstrated ability to collaborate effectively with physicians, nursing, rehabilitation, and administrative staff.
Independent and self-directed with confident, decisive clinical judgment.
Effective communicator with patients, families, and interdisciplinary teams.
Reliable, punctual, and hardworking; detail-oriented with strong documentation habits.
Proactive and assertive in clinical management; adaptable to fast-paced, diverse clinical environments.
Passionate about elderly care and chronic disease management.
Comfortable discussing sensitive topics, including goals of care and palliative needs.
Strong team player and collaborator.
