Saba Health Care - Restorative Nurse (RN/LPN)

, you’ll play a key role in enhancing the quality of life for our residents by developing and leading individualized restorative care plans focused on mobility, independence, and dignity.
- Uphold our commitment to high-quality, person-centered care.
- Support patients, families, and staff through ongoing communication, education, and recognition.
- Develop, implement, and evaluate the Restorative Nursing Program.
- Collaborate with the interdisciplinary team to establish and maintain restorative care standards.
- Ensure all restorative nursing practices comply with applicable laws and national standards.
- Provide or supervise direct restorative care services.
- Perform fall, restraint, and restorative assessments upon admission, quarterly, and as needed.
- Complete thorough and timely clinical documentation for all restorative care services.
- Work alongside the Director of Nursing to uphold clinical and regulatory standards of care.
- Carry out other duties as assigned in support of resident well-being and facility goals.
- Must be a currently licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Illinois.
- Knowledge of the Minimum Data Set (MDS) process required.
- Strong interpersonal, communication, and organizational skills.
- Proficient in electronic documentation and computer use.
- Ability to work collaboratively and compassionately within a team-oriented environment.
