Home Health Nurse

Chicago Home Healthcare. S.C. is seeking a field Registered Nurse with home health experience. We cover Chicago and surrounding suburbs.

The Home Health Registered Nurse (RN) provides professional nursing care to patients in their homes in accordance with physician orders and the established plan of care. The RN assesses patient health problems and needs, develops and implements nursing care plans, and maintains medical records while coordinating care with other healthcare professionals.

• Strong clinical foundation in Registered Nursing and Medicine, with the ability to perform comprehensive assessments and implement evidence-based care.
• Hands‑on experience in Home Care and Patient Care, including supporting activities of daily living and promoting patient independence.
• Proficiency in Wound Care, including dressing changes, monitoring healing, and recognizing signs of infection.
• Active, unrestricted Registered Nurse (RN) or Licensed Practical/Vocational Nurse (LPN/LVN) license in the state of Illinois.
• Current Basic Life Support (BLS) or CPR certification.
• Effective communication, documentation, and time‑management skills, with the ability to work independently in the field.
• Valid driver’s license, reliable transportation, and ability to travel to patient homes within the Chicago area.
• Prior home health or community nursing experience is preferred but not required; willingness to learn and adapt to varied home environments is essential.
