Salary range: $61.00–$79.67 per hour. Placement within the range depends on work experience, internal equity, and, where applicable, union agreements.

Northwestern Medicine is seeking an Advanced Practice Provider (APP) for our Comprehensive Stroke Center team at Northwestern Memorial Hospital. The APP will provide comprehensive clinical management for patients on the inpatient stroke services (no ICU) and work closely with attending physicians, fellows, residents, and medical students to deliver high‑quality, evidence‑based acute stroke care.

Schedule:

Monday‑Friday, 4×10‑hour days (day off to be determined) with possible occasional weekends. No nights or on‑call required.

• Provide direct patient care for individuals admitted to the primary stroke service, including assessment, diagnosis, treatment planning, and ongoing management.
• Flex to support stroke consult services as needed based on patient volume and program priorities, including running inpatient stroke codes.
• Assist with the management of medically complex stroke patients.
• Lead efforts on efficient patient throughput, including admissions, transfers, and discharges.
• Assist the team by coordinating consults, multidisciplinary care, and discharge planning.
• Ensure smooth transitions of care across inpatient and outpatient settings.
• Ensure accurate documentation and adherence to stroke protocols to support accreditation.
• Participate in quality improvement initiatives related to stroke care outcomes and compliance.
• Collaborate with interdisciplinary team members including attendings, fellows, and residents.

• Demonstrates clinical expertise and expanded knowledge through patient care activities which may include diagnosis and management of a wide range of health problems.
• Responsible for the health outcome of an assigned patient population.
• Confers with collaborating physicians and other health professionals in the area of clinical expertise.
• Develops appropriate educational systems for patients, families and staff in areas of expertise.
• Mentors graduate students in aspects of advanced practice and patient care.
• Actively participates in quality improvement and/or research pertinent to the area of expertise.

• Conduct comprehensive appraisals of patients' health status by taking health histories and performing physical examinations.
• Provide direct clinical care to patients.
• Prescribe/orders medications, therapies; order/interprets laboratory tests, x‑rays and procedures according to clinical privileges.

• Provide comprehensive teaching to patients and families.
