Compensation
Salary Range: $212,944 annually - $269,216 annually

What You Can Expect
The role of the Physician in a community setting is a response to societys demand for accessible, quality health care for all populations, regardless of socio-economic status.
Individual accountabilities and work volume will be established through the development of annual Success Objectives within the framework outlined below.
This position requires the ability and willingness to work some nights and weekend shifts as required to align with community health needs. The position may also require participation in mobile health initiatives.
Key Responsibilities
Patient Care and Clinical Practice
- Facilitate entry of the patient into the health care system by assessing primary health care needs
- Elicit comprehensive health histories
- Perform physical examinations
- Order and/or perform pertinent diagnostic tests
- Analyze collected data to determine client health status and formulate a problem list/diagnosis
- Provide coordinated, continuous, and culturally competent primary health care to clients across the health continuum through mutual goal setting and care planning
- Prescribe a variety of treatment modalities to meet health and illness needs
- Manage and evaluate the plan of care, process, and outcomes, then modify the plan as needed
- Coordinate services with other team members to meet patients primary health care needs and ensure continuity of care
- Document comprehensive, accurate, and continual data in client records and program reports according to Organization standards
Collaboration and Team-Based Care
- Collaborate with advanced practice nurses, community health nurses, and other medical providers to ensure health teaching and counseling is provided
- Collaborate with multidisciplinary professionals and community agencies to provide high-quality care
- Practice collaboratively with advanced practice nurses and community health nurses to prevent duplication of services and meet Organization productivity requirements
- Consult with health team members regarding client treatment plans
- Provide clinical supervision of medical care delivered by Physician Assistants, PA-Cs
- Serve as a collaborating physician to Advance Practice Nurses, APNs
- Follow clinical practice standards, protocols, and responsibilities related to each health care providers role
Productivity, Access, and Continuity of Care
- Meet average monthly provider productivity standards, including but not limited to number of visits
- Follow role assignments aligned with Patient Centered Medical Home standards
- Participate in all Patient Centered Medical Home efforts
- Assure access and continuity of care by providing call for after-hours coverage
- Assure continuity of care by obtaining hospital privileges at designated hospitals
