Part-Time Licensed Practical Nurse Hours

Mon 1p-7p, Thu & Sun 7a-1p. Additional Per Diem shifts available.

Elderly male with progressive dementia, severe fall risk. Uses wheelchair. Requires a nurse who makes a great first impression. Memory care experience a must.

• Perform care as defined by client’s individual Plan of Care and within scope of the state’s nurse practice act.
• Monitor patient’s condition and notify appropriate personnel of any changes.
• Provide those services requiring substantial and specialized nursing skill.
• Assure proper maintenance of clinical records in compliance with local, state, and federal laws.
• Counsel the client and family in meeting nursing and related needs.

• Current and unrestricted LPN license in the state for which you are applying (Illinois).
• Home health experience preferred.
• Memory care experience required.
• Minimum 1 year experience.
• Proof of eligibility to work in the U.S.
• Clean background and criminal record check.
