Job Description IMAN (Inner-City Muslim Action Network) is a Muslim-led community health organization operating a primary care clinic on Chicago's south side, serving predominantly low-income, uninsured, and immigrant patients in the Chicago Lawn neighborhood. We seek a full-time Nurse Practitioner with Family Nurse Practitioner (FNP) or Women's Health Nurse Practitioner (WHNP) certification to join our this role, you will manage an independent patient caseload with full autonomy in clinical decision-making.
Your focus will be on wellness promotion, disease prevention, patient education, and culturally responsive care tailored to our diverse community. You'll conduct comprehensive patient assessments and health histories, order and interpret diagnostic tests, develop individualized treatment plans, and prescribe medications. Procedural responsibilities include IUD placement and Nexplanon insertion. You'll provide health education to patients and families, maintain thorough EMR documentation, and contribute to a patient-centered care model.###
Required Qualifications
* MSN or DNP degree from an accredited nursing program
* Active Illinois Registered Nurse (RN) license
* Active Advanced Practice Registered Nurse (APRN) license in Illinois
* National board certification: ANCC or AANPCB for FNP;
National Certification Corporation (NCC) for WHNP
* Current BLS certification
* Illinois Controlled Substance License
* DEA registration
* Proficiency with electronic medical records (EMR)###
Preferred Qualifications
* Full Practice Authority authorization in Illinois
* At least one year of clinical practice experience
