Position

Licensed Practical Nurse II (FTE
1). M-F 8-5, some Sat. Day shift. Salary $26.99/hr to $41.93/hr requires Illinois residency within 180 days of hire and valid LPN license.

Position Summary

Licensed Practical Nurse II is accountable for the delivery of patient care as delegated by and under the direction of a physician or registered nurse utilizing the Nursing Process and Relationship-Based Care model. The LPN II provides direct patient care and improves outcomes through shared leadership, evidence‑based practice, and standards of excellence.

Duties & Responsibilities
  • Complete required orientation program and unit‑specific competencies.
  • Participate in patient assessment as delegated by the RN, observing, collecting, recording, and reporting patient data.
  • Contribute to a plan of nursing care with the RN based on ongoing observations and input from patients and families.
  • Implement orders from physician, PA, or APRN as delegated by the RN and document accurately in the electronic medical record.
  • Share information and collaborate with healthcare team members to promote effective patient care.
  • Recognize symptoms, establish priorities, and respond to patient’s changing conditions and needs.
  • Engage and obtain information from patients and/or families to support an appropriate educational plan.
  • Demonstrate commitment to UI Health values through teamwork, communication, and quality outcomes.
  • Conduct screening tests for common diseases/viruses.
  • Perform other duties as assigned.
Professional/Leadership
  • Advocate for patients, families, and the community.
  • Integrate evidence‑based research, current standards, and clinical experience to provide quality patient care.
  • Support shared leadership as the nursing governance method.
  • Participate in organizational and departmental activities supporting unit goals.
  • Maintain accountability for job performance and required certifications (LPN licensure, BLS, unit‑specific).
Organizational
  • Adhere to the Medical Center’s Code of Conduct and respect patients, families, and staff.
  • Follow nursing time, overtime, and attendance policies.
  • Complete electronic learning modules by established dates.
  • Comply with TB, flu, and fit‑testing requirements.
  • Utilize established communication channels to resolve patient care problems.
  • Identify recurring clinical issues and participate in problem‑solving.
Education & Research
  • Collaborate with nurses to provide positive learning experiences for staff and students.
  • Serve as a resource for nursing staff and students.
  • Access the intranet for relevant communications and policy changes.
  • Support nursing and multidisciplinary research initiatives.
  • Engage in professional growth by identifying learning needs and pursuing strategies.
Reporting Structure

Works within the scope of practical nursing as defined by the Illinois Nurse Practice Act, under the direction of a physician or RN. The Director provides coaching, guidance, and performance evaluation.

Minimum Qualifications
  • Graduation from an accredited Licensed Practical Nurse program.
  • Eligibility for licensure and possession of a valid license issued by the Illinois Department of Financial and Professional Regulation.
  • At least six months of nursing experience in a healthcare facility.
Preferred Qualifications
  • Effective oral and written communication facilitating therapeutic relationships.
  • Basic computer skills and willingness to become proficient in Epic.
