ACT Psychiatric Nurse (RN)

Founded in 1948 in Chicago, Envision Unlimited provides a full spectrum of care for individuals with disabilities ranging in age from infancy to 80+. We serve all individuals regardless of race, gender, religion, or ability to pay. Our innovative services span Chicagoland and Central Illinois and include community living, day programs, behavioral health, employment services, family respite, foster care, and supportive housing.

We are seeking a compassionate Registered Nurse (RN) to join our Assertive Community Treatment (ACT) team. In this role, you'll provide community-based psychiatric nursing services to adults living with serious mental illness while collaborating with an interdisciplinary team dedicated to promoting recovery and independence.

Responsibilities

• Coordinate medical care and annual health evaluations for ACT clients.
• Administer injectable psychotropic medications under psychiatric direction.
• Monitor lab results, vital signs, medications, and medical appointments.
• Coordinate with physicians, pharmacies, hospitals, and community providers.
• Conduct home visits as needed and respond to medical emergencies.
• Participate in treatment planning, clinical meetings, and maintain timely Medicaid Rule 132/140 documentation.
• Provide education and support to clients, caregivers, and staff.

Qualifications

• Current Illinois Registered Nurse (RN) license.
• Minimum of 2 years' experience working with individuals with serious mental illness.
• Valid Illinois driver's license and ability to travel within the community.
• Ability to pass a background check and complete required annual training.
