The Transplant Nurse Coordinator (CNCII) is a Registered Nurse with experience in transplantation and living solid organ donation. Under the supervision of transplant surgeons and physicians, the transplant Nurse Coordinator coordinates the clinical aspects of transplant patient and living donor care throughout the pre-transplant phase. CNCII facilitates patient care delivery using the Nursing Process and Relationship-Based Care framework. CNCII provides patient care and consultation, coordinates staff education, and acts as a clinical role model.

The CNCII aims to improve patient outcomes by integrating Shared Leadership and Evidence-Based Practice.

• Evaluate patients at the appropriate UI Health site of care.
• Coordinate care for transplant recipients from referral through wait listing, all potential recipients with living donors, and all living donor patients.
• Ensure pre-transplant patient coordination and education are documented and compliant with UNOS, CMS transplant program guidelines, and UI Health policies/procedures.
• Collaborate with the provider to order necessary testing for evaluation and review results in a timely manner following UNOS and CMS guidelines.
• Develop, coordinate, and communicate the patient’s plan of care based on ongoing assessment and family input.
• Use clinical expertise to collaborate with the multidisciplinary health care team to improve patient outcomes.
• Consult with and implement orders by performing provider-prescribed interventions and document accurately in the EMR.
• Obtain clinical information from the multidisciplinary team, patient, and/or family to develop an appropriate educational plan.
• Recognize symptoms, establish priorities, and respond to changing patient needs throughout the transplant care continuum.
• Ensure the patient is added to the UNOS waiting list and that listing letters are sent per UNOS requirements.
• Coordinate and schedule all living donor transplant surgeries as assigned.
• Attend and participate in transplant QAPI meetings, listing meetings, department meetings, and other relevant meetings.
• Provide cross-coverage for the transplant data coordinator by entering living donor information in UNOS prior to surgery as assigned.
• Engage nursing and support personnel by delegating appropriate tasks per the Illinois Nurse Practice Act and provide support and feedback.
• Demonstrate commitment to UI Health values through teamwork, communication, quality outcomes, patient safety, and a clean, safe environment.
• Integrate theoretical knowledge, evidence-based research, current standards of practice, and clinical experience to deliver quality patient care.
