Fertility Centers of Illinois is seeking a full-time Nurse Case Manager for its on-site location in Chicago, IL. The Nurse Case Manager will manage patient care by coordinating with healthcare providers, monitoring treatment plans, and guiding patients through their fertility journey.

• Coordinate with healthcare providers to manage patient care and treatment plans.
• Monitor treatment plans and progress; guide patients through their fertility journey.
• Perform case management and utilization management.
• Discharge planning and collaboration with medical teams to ensure seamless care delivery.
• Provide patient education and clear communication regarding treatment progress.
• Offer emotional support in a compassionate and professional environment.

• RN required; BSN preferred
• Current state RN license without limitations required
• IVF/Fertility experience and background in women's health preferred
• Ability to manage a caseload of patients
• Computer proficiency required
• Excellent interpersonal skills and ability to work as part of a multi-disciplinary team and build and maintain effective working relationships
• Strong customer service and results oriented; highly responsive to requests
