Gentiva Hospice in Chicago is seeking a dedicated Hospice RN / RN Case Manager to lead patient care at home and coordinate with an interdisciplinary team. You will perform comprehensive RN assessments, implement and document plans of care, and educate families on end-of-life care and symptom management.
This full-time role requires at least 1 year of RN experience (hospice or home health preferred), CPR certification, and strong communication.
