Vascular Nurse Practitioner Job Responsibilities:

The Vascular Nurse Practitioner (NP) is responsible for providing comprehensive mobile vascular care to patients residing in skilled nursing facilities, rehabilitation centers, assisted living facilities, wound care centers, and outpatient locations. This position focuses on the evaluation and management of peripheral arterial disease (PAD), chronic limb-threatening ischemia (CLTI), venous disease, diabetic foot ulcers, and non-healing wounds.

The NP will work collaboratively with vascular specialists, wound care providers, primary care physicians, podiatrists, and nursing facility staff to identify patients requiring vascular intervention while helping prevent limb loss through early diagnosis and treatment.

  • Conduct comprehensive vascular and wound assessments, including physical examinations, pulse evaluations, Doppler studies, and noninvasive vascular testing (ABI, TBI, Doppler waveform analysis, and PVRs as available).
  • Evaluate and manage patients with peripheral arterial disease (PAD), chronic limb-threatening ischemia (CLTI), venous disease, non-healing wounds, diabetic foot ulcers, ischemic ulcers, edema, rest pain, gangrene, and tissue loss.
  • Assess wound characteristics, including measurements, drainage, infection, tissue viability, and healing progression, and identify patients requiring urgent vascular intervention.
  • Interpret diagnostic findings, document clinical assessments, and communicate significant abnormalities and treatment recommendations to supervising physicians.
  • Perform pre-procedural evaluations for vascular and endovascular interventions, including review of medical history, medications, anticoagulation management, laboratory studies, imaging, and procedural risk factors.
  • Coordinate care across the continuum with referring providers, primary care physicians, specialists, wound care teams, post-acute facilities, and home health agencies to ensure seamless diagnosis, treatment, and follow-up.
  • Serve as a primary patient liaison by providing education on vascular disease, treatment options, procedural recommendations, risks and benefits, and by facilitating communication with family members and healthcare proxies.
  • Complete accurate and timely electronic health record (EHR) documentation, including histories and physicals, consultation notes, wound assessments, medical necessity documentation, procedural clearances, follow-up visits, and care coordination activities.
  • Travel regularly to skilled nursing facilities, long-term care facilities, assisted living communities, rehabilitation centers, physician offices, and outpatient wound care centers.
  • Collaborate with multidisciplinary teams including interventional cardiologists, vascular specialists, vascular surgeons, podiatrists, wound care providers, nurses, therapists, and facility leadership.
  • Support limb preservation initiatives, vascular screening programs, quality improvement activities, and evidence-based practice while maintaining compliance with regulatory, accreditation, and payer requirements.

Vascular NP Qualifications and Requirements:

  • Preferred clinical experience in Vascular Medicine, Cardiology, Wound Care, Interventional Radiology, Interventional Cardiology, Vascular Surgery, Critical Care, or Internal Medicine, with demonstrated expertise in the assessment and management of complex vascular patients.
  • Preferred experience managing patients with Peripheral Arterial Disease (PAD), Chronic Limb-Threatening Ischemia (CLTI), Venous Disease, Diabetic Limb Preservation programs, and delivering care in mobile or community-based healthcare settings.
  • Master's or Doctorate Degree in Nursing required.
  • Current Nurse Practitioner license required.
  • Current DEA registration required.
  • Current BLS certification required.
  • ACLS preferred.
  • National Board Certification as Nurse Practitioner required.
  • Valid driver's license with reliable transportation.
