As the Wound Care Coordinator (RN) Registered Nurse, you will:
Perform comprehensive skin and wound assessments upon admission and as clinically indicated.
Develop, implement, and evaluate individualized wound care treatment plans.
Monitor wound healing progress and modify treatment plans based on resident response.
Coordinate wound care treatments, including dressing changes and specialized therapies.
Collaborate with physicians, nurse practitioners, therapy staff, dietitians, and the interdisciplinary care team to optimize resident outcomes.
Monitor residents for signs of infection or complications and communicate significant changes promptly.
Educate residents, families, and staff on wound prevention, skin integrity, and treatment plans.
Conduct routine skin rounds and participate in interdisciplinary care plan meetings.
Ensure compliance with all federal, state, and facility regulations related to wound care, infection prevention, and documentation.
Maintain accurate and timely documentation in the electronic medical record (EMR).
Track wound care outcomes and quality metrics to support continuous performance improvement.
Assist with Quality Assurance and Performance Improvement (QAPI) initiatives.
Serve as a clinical resource and mentor for nursing staff on wound care best practices.
Manage wound care supplies and ensure appropriate inventory levels.
On Call rotation - one weekend per month
Perform other duties as assigned.
Current, unrestricted Registered Nurse (RN) license in the State of Illinois.
Wound Care Certification (WCC, WOCN)
Minimum of one year of wound care experience required
Skilled nursing or long-term care experience strongly preferred.
Strong knowledge of wound care practices, pressure injury prevention, infection control, and regulatory compliance.
Excellent assessment, critical thinking, communication, and organizational skills.
Experience with electronic medical records (EMR).
