UChicago Medicine is hiring a Population Health Nurse Navigator to support the Care Transition Clinic at our Hyde Park campus. This 0.6 FTE, 100% onsite role requires a Chicago-area based RN with BSN and strong patient navigation skills.
The Nurse Navigator coordinates care across the continuum, triages needs, educates patients and families, and acts as a liaison among clinical teams to prevent readmissions and optimize population health outcomes.
