Harmony Cares is seeking a Nurse Practitioner to provide in-home primary care for high-acuity geriatric patients in NW Chicago. The role emphasizes patient-centered care with a Monday–Friday schedule, no weekends or evenings, and managing chronic conditions within a geriatric panel.
You will be supported by an integrated care team and receive a comprehensive benefits package, CME/licensing stipend, malpractice tail coverage, and Up To Date access. Experience in home health or geriatrics preferred.
