The Integrated Health Clinic Nurse performs registered nursing duties within Trilogy's integrated healthcare clinic and within the broader community. The role centers on assessment and triage, care coordination, and health education delivered in a culturally appropriate manner using motivational interviewing and other evidence-based strategies.
The nurse collaborates with an interdisciplinary treatment team to ensure quality standards are met and provides strengths-based, client-centered, trauma-informed care to clients with serious mental illness, substance use disorders, and medical comorbidities — guided by a harm reduction approach.
Responsibilities
- Partner with a designated Federally Qualified Health Center (FQHC) organization to deliver clinic nursing services and serve as a liaison between that partner and Trilogy staff.
- Assess clients' presenting health problems and triage for medical and psychiatric attention using accepted triage principles.
- Deliver individualized direct nursing care, including health education, care coordination, psychiatric and medical illness self-management coaching, and administering long-acting antipsychotic injections.
- Using established workflows refill psychiatric and medical medications for Integrated clinic providers as needed.
- Advocate for clients by communicating effectively with their Trilogy team members and linking them to needed community services.
- Collaborate with Trilogy and partner psychiatric providers to support treatment plans — including administering psychiatric rating scales, assessing medication adherence, and coordinating with external providers and family members.
- Work with the partner organization's primary care providers and clinic staff on patient plans of care to ensure quality standards are met.
- Support a population health management approach using client data on chronic conditions, health behaviors, and health literacy assessments.
- Provide logistical management and support for community-based programs.
- Plan and facilitate health education groups.
- Supervise and support nursing students and students from other disciplines as assigned.
- Perform other related duties and projects as assigned.
Qualifications
Required
- Bachelor's degree in Nursing or higher
- Valid Illinois driver’s license with daily access to a well-maintained vehicle with $100,000/$300,000 liability insurance
- BLS/CPR certification
Preferred
- Experience working with people with serious mental illness, substance use disorders, and/or medical comorbidities
- 1+ year of nursing experience
- Experience in community-based services
Skills
- Exceptional communication and customer service skills; ability to work effectively with a diverse range of partners
- Strong organizational skills with proficiency in Microsoft Word and Excel
