LVN LPN

Belmont Village Lincoln Park in Chicago, IL is seeking a compassionate LVN/LPN to provide high-quality care to assisted living and memory care residents. In this role, you will administer medications and treatments, monitor vital signs and chronic conditions, document care, and communicate with physicians, families, and interdisciplinary team members. You will support care plans, respond to resident needs and emergencies, and promote safety, dignity, and engagement.

Responsibilities

• Provide direct nursing care to assisted living and memory care residents
• Administer medications and treatments per physician orders and community policies
• Monitor vital signs, chronic conditions, and resident status, reporting changes promptly
• Document assessments, interventions, and outcomes accurately and timely
• Collaborate with physicians, families, and interdisciplinary team members on care plans
• Support, coach, and coordinate with caregiving staff to ensure quality resident care
• Respond to resident needs, urgent situations, and emergencies following protocols
• Promote resident dignity, safety, engagement, and a person-centered care environment
• Participate in training, quality improvement, and regulatory compliance activities
• Maintain infection control and safety standards throughout the community
