RN Case Manager

Inpatient Care Manager position available. The facility has a total of 205 beds and includes a Level II NICU. The unit size may vary with an average census of patients per day. This role primarily serves an underserved population, focusing on adults and geriatrics. The RN Case Manager will encounter complex cases, including those involving unhoused and low-income patients. Assignments may include areas such as PCU, Med Surg, and LDRP, with the possibility of floating to other units.

The typical patient caseload will be around 14-20 patients. The role requires working with a multidisciplinary team and physicians. Candidates must be comfortable asking questions and navigating various insurance plans. Assistance with facility initiatives will be part of the duties. Soft skills are essential for this role, particularly those related to assessment and resource finding. Support will be available within the department from leadership, other case managers, and hospitalists.

Technology and equipment used include EMR systems and referral platforms. The schedule includes a weekend rotation every fifth weekend. There are no on-call requirements for this position. Holiday expectations are set at 50% or two holidays within the contract period. The standard shift times are Monday to Friday, from 0800 to 1630. If working on a weekend, a Monday and Friday off will be offered afterward.

A set schedule with weekend rotation will be provided. Scrub color will be eggplant, with business casual attire and a white lab coat required.
