About the Role

We are seeking a motivated and dedicated Home Health LPN to join our team in Chicago, IL. As a Home Health LPN, you will be responsible for providing direct patient care, administering medications, and performing various medical procedures. You will also work closely with other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities
  • Provide direct patient care, including vital sign checks, wound care, and other medical procedures as ordered by the physician
  • Administer medications and treatments as prescribed by the physician
  • Perform routine tests and observations to monitor patient status and report any changes to the nurse supervisor
  • Develop and implement individualized care plans that promote patient independence and well-being
  • Collaborate with other healthcare professionals to ensure seamless coordination of care
  • Maintain accurate and timely records of patient care and outcomes
  • Participate in ongoing education and training to stay current with industry developments and best practices
Requirements
  • Current LPN licensure in the state of Illinois
  • Minimum 1 year of experience in home health care (new graduates welcome to apply!)
  • Ability to work independently and as part of a team
  • Strong communication and interpersonal skills
  • Ability to lift and move patients and equipment as needed
  • Reliable transportation and willingness to travel to patient locations
