As a Home Health RN with Orion Home Health, you will play a vital role in providing high-quality, individualized care to patients in their homes, promoting independence, comfort, and well-being. • Conduct initial patient assessments, develop personalized care plans, and provide skilled nursing care to patients in their homes
• Collaborate with interdisciplinary team members, including physicians, therapists, and social workers, to ensure comprehensive care
• Administer medications, perform wound care, and provide education to patients and their caregivers
• Monitor patients' progress, report changes, and make recommendations for adjustments to care plans
• Maintain accurate and detailed records of patient interactions, treatments, and progress

• Current registration with IDFPR as a Registered Nurse (RN)
• Valid Driver's License (DL) and reliable transportation
• Experience:
  
  Home health, acute care or skilled nursing preferred
• Level of
  
  Education:
  
  ADN or BSN
• Ability to drive to patient home across assigned territory. Ability to work in varied home environments
• Proficiency with EMR systems.
