Position: NURSE PRACTITIONER - DECATUR, IL

Location:

  • Mobile community health events throughout the Decatur area
Key Responsibilities School & Sports Physicals
  • Conduct comprehensive school-required physical examinations for children and adolescents.
  • Conduct sports physicals and assess patients for participation clearance when appropriate.
  • Review medical history, medications, allergies, previous diagnoses, and relevant health records.
  • Perform age-appropriate physical assessments and document findings.
  • Identify health concerns requiring additional evaluation or follow-up.
  • Complete required school, sports, and physical examination documentation.
  • Provide families with guidance regarding recommended follow-up care when needed.
Vaccinations & Immunization Services
  • Prepare and administer school-required and other approved vaccines.
  • Review immunization histories and identify missing or overdue vaccinations.
  • Review consent forms and relevant patient information before vaccine administration.
  • Educate patients and parents regarding required and recommended immunizations.
  • Monitor patients following vaccination and respond appropriately to clinical concerns.
Preventive Health & Patient Care
  • Perform preventive health screenings, including blood pressure, glucose, cholesterol, and other event-specific testing.
  • Collect patient specimens and perform procedures such as phlebotomy, nasal swabs, and vital-sign assessments, as required.
  • Review patient information and document services in the electronic health record system.
  • Provide patient education regarding physical exam findings, vaccines, screening results, preventive care, and healthy lifestyle practices.
  • Answer patient and parent questions regarding services, results, follow-up care, and treatment recommendations.
  • Refer patients for additional medical evaluation or follow-up when needs fall outside the services available at the event.
Staff Leadership & Education
  • Assess, plan, and implement competency assessments and continuing education.
  • Lead orientation and onboarding of new clinical staff.
  • Conduct in-services and protocol trainings.
  • Oversee Medical Assistants and provide backup support as needed.
  • Provide input on staff performance reviews and corrective action plans.
  • Stay current with clinical literature and evolving public health guidance.
  • Implement process improvements aligned with organizational goals.

Qualifications & Skills
  • Strong experience conducting physical examinations
  • Strong clinical assessment and decision-making skills
  • Experience administering vaccines
  • Excellent verbal and written communication
  • Ability to work effectively with children, adolescents, and families
  • Adaptability in dynamic/mobile environments
  • Infection prevention & disease control knowledge
  • Strong organizational skills
  • Proficient with EMR systems and medical documentation

Education, Experience & Licensing
  • Active Illinois Nurse Practitioner license (Required)
  • BLS Certification (Required)
  • Experience conducting school and/or pediatric physical examinations preferred
  • Experience administering pediatric and adolescent vaccines preferred
  • Experience with EMR systems and medical terminology
  • Experience in mobile clinics, school-based health, pediatrics, or community health preferred
  • Spanish-speaking ability preferred (not required)
Salary range
  • $60 - $65 per hour
