Job Summary:
Alpine Care of St. Charles is seeking a dedicated and experienced MDS Coordinator RN/LPN to join our team in St. Charles, IL. Responsibilities:- Conduct MDS assessments and reviews to ensure accurate and comprehensive data collection and reporting
- Coordinate with interdisciplinary team members to gather information and complete MDS assessments
- Ensure that all MDS assessments are completed in a timely and thorough manner, meeting all regulatory and accreditation requirements
- Develop and implement effective strategies to improve MDS accuracy and completeness
- Provide education and training to staff on MDS processes and procedures
- Maintain accurate and up-to-date records and reports
