Job Summary

Case Managers/FSM partner with program participants and families to build mutually trusting relationships, serve as an advocate for families, and provide support services as needed. The Case Managers/FSM support families and program participants in prioritizing their needs and developing goals to achieve long-term success. Additionally, the Case Manager/FSM will identify, develop, and maintain community partnerships and resources that promote health equity and mental wellness.

Scope of Responsibilities
  • In collaboration with Outreach teams, CMs/FSS recruit participants and family caseloads.
  • Program Participant caseload is based upon Outreach team’s caseload.
  • However, each case manager should have a minimum of five families to support.
  • Provide case management services to families and program participants, including direct services to families by assisting them in determining family strengths, needs, and goals.
  • Conduct initial and ongoing assessments of family needs; develop resources; provide referrals and supportive services to assist families in meeting identified needs; and document and report on all social service activity, as required.
  • Identify and develop resources to meet identified health, social service, and mental health needs; develop effective working relationships with related and relevant resources.
  • Participate in regularly scheduled team meetings to plan for and deliver collaborative services.
  • Meet regularly with program participants, families, stakeholders, and other staff regarding the progress of family and program participants.
  • Conduct home visits for purposes of assessment and support.
  • Discreetly handle sensitive information and maintain confidentiality at all times.
