Role overview

In this meaningful and dynamic role, the Care Manager Supervisor brings together direct client care and team leadership to make a lasting impact on the lives of Holocaust survivors. You’ll maintain an active caseload, providing compassionate, trauma-informed support that helps older adults live with dignity, independence, and connection.

At the same time, you’ll mentor and guide a small team of Care Managers—offering clinical insight, support on complex cases, and day-to-day supervision to ensure high-quality, person-centered care. This is an opportunity to both deepen your clinical skills and grow as a leader, while contributing to a supportive team dedicated to serving a resilient and deserving community.

This is a hybrid and flexible role as appropriate, with occasional programming outside of regular work hours.

What You’ll Do
  • Care Management
    • Maintains an active client caseload, providing clinical, trauma-informed case management and conducting comprehensive in-home, office-based, and remote assessments of survivors’ psychosocial, financial, functional, and environmental needs.
    • Determines eligibility and services in accordance with program and funder guidelines, including evaluation of eligibility and available benefits.
    • Provides ongoing case management, resource coordination, advocacy, and support to survivors and their families.
    • Responds to and coordinates service requests in alignment with program standards and funder requirements.
    • Monitors services and adjusts supports as client needs, eligibility, or circumstances change.
    • Completes timely and accurate documentation in accordance with program, funder, and regulatory requirements.
    • Participates in weekly supervision, case consultation, and interdisciplinary team meetings.
    • Contributes to program planning and implementation, including cultural and social programs such as Café Europa, commemorations, and holiday events.
    • Provides counseling, facilitates support groups, and participates in community education or professional presentations as appropriate to training and experience.
  • Care Management Supervisor
    • Provides supervision to assigned Care Managers, including weekly one-on-one supervision meetings and ongoing case consultation.
    • Reviews and approves client assessments and service determinations to ensure consistency, quality, and compliance with program and funder guidelines.
    • Supports Care Managers in navigating complex cases involving trauma history, cognitive decline, medical needs, financial insecurity, social isolation, and family dynamics and provides consultation and problem-solving as needed.
    • Guides staff in assessment-based decision making, including eligibility determination, service authorization, and benefits navigation.
    • Supports staff development, job performance, and adherence to agency and program standards and documentation requirements.
    • Serves as backup supervisor for additional staff as needed.
    • Participates in Supervisors’ Meetings and contributes to program-level planning and quality improvement.
Compensation

$75,000 to $85,000 commensurate with experience, with $2,500 sign-on bonus.

Schedule

Monday to Friday, 8:30 am to 5:00 pm with a hybrid and flexible schedule as appropriate, with occasional weekend or evening attendance based on programs.
