Position: EL RESCATE CASE MANAGER

Reports to: El Rescate Program Coordinator

Program/Department: El Rescate

Classification: Exempt

Revision Date: February 2023

Summary of Position

The Case Manager will be responsible for providing direct services to the target population and reports to the Program Coordinator. The Case Manager provides individual case management, follow-up services, career counseling, job search, health care linkage, and more.

Essential Duties and Responsibilities
  • Support staff in managing the development and implementation of service plans and the clinical coordination of services to clients through auditing of client files
  • Assist with maintenance of all youth program documentation, including client data and all reports required by all funding sources
  • Participate in on‑ and off‑site staff training, workshops, and outreach events as needed
  • Provide intake and assessment, follow‑up services
  • May present information regarding youth programs and services to community groups and organizations
  • Determine eligibility and assist youth with enrollment into program services
  • Assist clients with their career planning and career decision‑making process
  • Assist clients in identifying and addressing barriers to employment, and refer services as appropriate to community resources as necessary
  • Complete and submit all necessary case notes, assessments, and individual service plans
  • Attend all weekly staff program (El Rescate) and monthly staff organization (PRCC) mandatory meetings
  • Participate in El Rescate's fundraising events
  • Provide short‑term case management, including budgeting assistance, service planning, advocacy, and referrals
  • Attend the PRCC and program‑specific and organizational activities and meetings, including but not limited to the monthly organization‑wide General Assembly
  • Participate in organizational annual events including but not limited to the Puerto Rican People’s Parade, Fiesta Boricua, Three Kings Day, and Haunted Paseo Boricua
  • Perform other functions assigned
Compliance

This position requires compliance with all of The PRCC’s written standards, including its Standards of Conduct, all policies and procedures, and Regulatory Compliance requirements. Compliance will be considered part of regular performance evaluation.

Requirements
  • Ability to interact positively with a variety of personalities and socioeconomic populations
  • 2 years of youth program and/or counseling, case management, monitoring, or related experience (required)
Qualifications
  • Bilingual — English/Spanish preferred
  • Excellent interpersonal, verbal, and written communication skills
  • Ability to work independently with minimal guidance
  • Task‑oriented, non‑judgmental, open personal qualities, and clear sense of boundaries
  • Ability to work in a team environment and provide support to fellow team members
  • Proficiency in Microsoft Office Suite, database management, spreadsheet applications, email, and Internet applications
Education

Bachelor's degree in career counseling, educational/guidance counseling, Human Services, or a related field.

Working Conditions and

Physical Requirements

OSHA Category 3 – no regular exposure to blood, body fluids, or tissues; tasks involving such exposure are not part of employment.
