Social Services Director – Responsibilities

Prepare residents for their future by ensuring their social and emotional needs are met during their stay. Coordinate resident discharges and participate in resident care conferences. Guide residents and families through the transition process and maintain accurate documentation.

• Bachelor’s degree in Social Work or a related field.
• Minimum of two (2) years of experience as a social worker, preferably in a healthcare setting.
• Prior Social Services Director experience in a skilled nursing facility is preferred.
• Graduate degree in social work or related field with five (5) years of related experience is advantageous.
• Strong communication, leadership, and organizational skills.
• Ability to work collaboratively with interdisciplinary teams.
