Social Services Designee – Skilled Nursing Facility

Harmony Chicago, a Gold Seal of Approval® certified post‑acute care facility located in Albany Park, Chicago, seeks a Social Services Designee to support residents’ social and emotional needs.

• Prepare residents for transitions and discharges.
• Coordinate care conferences and discharge planning.
• Provide ongoing social and emotional support to residents.
• Work collaboratively with interdisciplinary team.

• High school diploma or equivalent.
• Minimum 2 years experience in a Social Worker role, preferably in a healthcare setting.
• Prior experience as a Social Services Designee in a skilled nursing facility preferred.
• Bachelor’s degree in Social Work or related field and 5 years related experience preferred.
