Summary

The Social Work Care Mgr (CM) works with physician practices and inpatient teams to promote the effective utilization of services and coordination of patient‑centered care. The CM contributes to the team's effectiveness by reviewing plans of care, identifying barriers to efficient utilization of resources, and appraising patients' psychosocial, financial, and clinical needs throughout the continuum of care. The CM functions as a liaison between patients, physician practices, the hospital, and the community, ensuring continuity of care and psychosocial needs are met.

Required Job Qualifications
  • Current license in Illinois as a Social Worker (e.g., LSW).
  • Ability to perform all job components and serve as a team resource for socially or clinically complex cases within the field of Social Work.
  • One year of experience (or completed internship) as a healthcare or mental health services provider for neonate, pediatric, adolescent, adult, and/or geriatric patients, with expert knowledge in case management, discharge planning, social services, and criteria application.
  • Ability to perform tasks independently, prioritize workload, and analyze data.
  • Experience with information systems and Windows technology and competency within the UM Information System.
  • Team‑building skills and flexibility.
Preferred Job Qualifications
  • Master’s in Social Work from an accredited university.
Physical Demands
  • Role requires walking and climbing stairs throughout the Medical Center.
Responsibilities
  • Functions as an expert clinical practitioner within the Case Management team and serves as a resource to assigned physician practice groups, sharing expertise as a social worker and supporting other CMs with socially complex cases.
  • Provides basic assessment of patients’ health‑care needs, monitors progress, and confers with physician groups and the health‑care team regarding variances from the anticipated plan of care; seeks assistance as needed for complex clinical issues.
  • Provides and coordinates social services to patients and families; assesses patients’ basic and in‑depth psychosocial, financial, cultural, and family situations.
  • Manages a complex patient caseload and demonstrates expert ability to coordinate and implement discharge management services, ensuring continuity of care planning; finds community resources, maintains contact with them, and consults with physicians and inpatient groups regarding use of those resources.
  • Reviews admissions and ongoing stays for medical necessity, appropriateness, and quality of care; applies criteria and performs concurrent review processes; works with physicians and the Medical Director on case issues and concurrent denials as indicated.
  • Applies pathways/guidelines, assesses variances, and proposes interventions; participates in developing, implementing, and evaluating pathways/guidelines and process improvement plans.
  • Collaborates with inpatient staff and physician groups to determine goals for length‑of‑stay management and discharge activities; promotes cost‑effective quality care services.
  • Monitors and reports potential quality issues and makes recommendations when appropriate.
  • Utilizes the CM information system and appropriate technologies; monitors variances and trends related to select goals; assists the CM team and hospital groups in developing action plans and addressing opportunities for improvement.
  • Promotes customer satisfaction among patients, families, physicians, external case managers, payors, vendors, and inpatient staff; actively promotes ICARE principles.
  • Serves on hospital‑wide and departmental committees when applicable.
