UCAN Chicago’s Teen Parenting Service Network seeks a TPSN Transition Coordinator in Chicago, IL. The role acts as liaison to agencies serving pregnant and parenting youth in DCFS care, coordinating C21 processes, and supervising interns as needed.
You will lead projects, manage data, provide clinical consultation, and build relationships with DCFS and community partners. A Master’s degree in social work or related field and Illinois driver’s license required.
