POSITION

SUMMARY

The SC2 Workforce Case Manager is responsible for supporting participants from intake and enrollment through program completion within the Scaling Community Violence Intervention (SC2) Workforce and Job Development Program. The position provides participant‑centered case management, coordinates supportive services and referrals, maintains accurate program documentation, and helps participants address barriers that may affect engagement, workforce readiness, and employment success.

The Case Manager works collaboratively with the Program Manager and Employment Specialist in a cohort‑based workforce development environment. This role requires professionalism, sound judgment, strong organization, attention to detail, effective communication, and the ability to manage responsibilities independently while contributing to a collaborative team.

This position is subject to continued grant funding availability, program performance requirements, and organizational needs.

ESSENTIAL DUTIES AND RESPONSIBILITIES Participant Intake, Enrollment & Assessment
  • Conduct participant intake, enrollment, orientation, and related onboarding activities.
  • Receive and process participant referrals in accordance with program procedures.
  • Complete participant assessments and all required enrollment and program documentation.
  • Create, organize, and maintain complete participant files from intake through program completion.
  • Identify participant strengths, goals, barriers, service needs, and other factors that may affect successful program participation.
Case Management & Participant Support
  • Provide ongoing case management services and individualized participant support throughout program participation.
  • Maintain regular communication with assigned participants to promote engagement, attendance, accountability, and successful completion of program activities.
  • Monitor participant attendance, engagement, progress, and accountability throughout the cohort.
  • Identify barriers to participation or employment and coordinate appropriate supportive services, internal resources, and community referrals.
  • Follow up on referrals and participant needs to support continuity of services and progress toward program goals.
  • Maintain professional boundaries, protect participant confidentiality, and provide culturally responsive, participant‑centered service.
Program Operations & Cohort Support
  • Support the daily operations of the cohort‑based workforce development program in collaboration with the Program Manager and Employment Specialist.
  • Assist with participant orientation, scheduling, program activities, workshops, and other cohort needs as assigned.
  • Collaborate with the Program Manager and Employment Specialist to promote participant success, employment readiness, successful transitions to employment, and achievement of program outcomes.
  • Participate in case reviews, staff meetings, supervision sessions, trainings, departmental initiatives, and organizational meetings as assigned.
Documentation, Data Management & Compliance
  • Maintain thorough, accurate, complete, and timely case notes, attendance records, assessments, participant contacts, referrals, service documentation, and other required records.
