Envision Unlimited seeks a QIDP Case Manager to support individuals with intellectual, developmental, and psychiatric disabilities in community-based settings. In this role, you will coordinate person-centered services, develop and monitor individualized support plans, and advocate for client rights, inclusion, and independence. You will collaborate with families, clinical teams, and community partners, maintain accurate documentation, and ensure regulatory compliance. Responsibilities

  • Provide person-centered case management for individuals with intellectual, developmental, and psychiatric disabilities
  • Develop, implement, and monitor individualized service and support plans (ISPs)
  • Coordinate interdisciplinary services including residential, day, employment, and clinical supports
  • Conduct regular home and program visits to assess client progress and well-being
  • Maintain accurate, timely documentation in compliance with state and agency standards
  • Advocate for client rights, community integration, and access to resources
  • Collaborate with families, caregivers, and community partners to support client goals
  • Participate in team meetings, trainings, and quality improvement initiatives
  • Ensure compliance with QIDP regulations and funding requirements
  • Respond to crises and help develop behavior and safety support strategies
