PCC Community Wellness Center seeks a Recovery Social Work Care Manager (CADC-preferred) to coordinate screening and interventions for patients with complex chronic care needs at the ARC Recovery Services Clinic in Chicago. This licensed social worker connects patient needs with PCC ARC and hospital resources to support whole health outcomes.
The role collaborates with a multidisciplinary team, provides behavioral health support, and maintains care coordination across settings.
