Per diem Medical Social Worker providing direct and indirect services to patients and families. Requires daytime availability with weekdays preferred.

• Assist patients and families through adjustment to illness or hospitalization.
• Provide protective services for children and families at risk.
• Help families understand and adhere to treatment plans.
• Link patients and families to community agencies and services.
• Coordinate service delivery with families and appropriate team members across the continuum.
• Make necessary referrals to team members and community resources.
• Collaborate with families to utilize community service linkages.
• Meet daily with the inpatient floor case manager to review census and proactively identify barriers to discharge.
• Identify, assess, and respond to child/family needs, including developmental, psychosocial, cultural, and spiritual needs.
• Address social and emotional needs that impact medical treatment and outcomes.
• Develop trusting relationships with families for the patient’s well‑being and treatment.
• Provide timely intervention for families in emergency or crisis situations.
• Document thoroughly and timely in accordance with professional standards.
• Communicate with the health‑care team to provide optimal care of patients and families.
• Assist case managers in discharge planning and community linkages for families with complex needs.
• Maintain professional boundaries within social work and health‑care teams.
• Participate in interdisciplinary rounds, group problem‑solving, and program planning.
• Collaborate to develop programs targeting high‑risk families and to promote preventive health care.
• Educate interdisciplinary team on the impact of social and emotional issues on family coping.
• Lead family service professionals in establishing family‑services goals.
• Perform administrative and project‑management duties as assigned.
• May precept social work students and maintain social‑work intake and on‑call systems.
• Maintain and enhance referral lists for staff use.
• Communicate team‑level policy changes that impact children and families.
• Adhere to service principles with a customer‑service focus.

• Master’s degree in social work from a university approved by the Council for Social Work Education.
• Current Illinois State Social Work licensure (LSW) required; LCSW preferred.
• Work or internship experience in a health‑care, child welfare, or mental‑health setting preferred.
• Experience working with children and families.
• Ability to think proactively, creatively, and take initiative appropriately.
• Excellent communication, critical‑thinking, and interpersonal skills.
• Strong teamwork and interpersonal collaboration across multiple teams.
• Spanish language abilities strongly preferred.
• Stamina and energy to maintain direct patient contact on a regular basis with seriously ill persons.
• Capacity to coordinate multiple demands within time constraints and fulfill on‑call requirements.

Hourly Pay Range: $28.50–$46.60. Pay range is reviewed regularly to remain aligned with market conditions.
