POSITION SUMMARY

The Case Manager Supervisor provides direct supervision of all Transitional Housing Case Managers, Family Housing and Permanent Support Housing Case Managers. Work includes collaboration with other departments and leaders, guest grievance resolution, training and direction to staff engaged in case management, tenant support and resource acquisition on behalf of those served in Breakthrough’s program.

ESSENTIAL RESPONSIBILITES/FUNTIONS
  • Supervise, manage, monitor, train and coordinate the activities of all assigned staff using approved performance management processes and policies.
  • Knowledge, implementation & ability to train team on all service delivery models
  • Provides detailed statistical and narrative reports to the Program Director.
  • Attends meetings with related external program partners and internal departments.
  • Completes reports for funding agency on a routine basis.
  • Ensures all staff data is entered into the database daily.
  • Establishes and implements a system of internal quality management tracking and monitoring
  • Independently follows agency policy and procedures
  • Complete any other related tasks and responsibilities as assigned and required
  • Follows through with commitments including meeting established timelines
  • Identifies and resolves difficult issues with staff and/or the Director of Housing Programs; and develops and implements immediate corrective action plans.
