Family Support Partner

Program Overview

In partnership with the Oregon Health Authority, Youth Villages provides community‑based, multidisciplinary support to youth from birth to age
18 through the Intensive In‑Home Behavioral Health Treatment (IIBHT) program. Staff work alongside youth and families to deliver individualized services including case management, care coordination, psychiatric care, individual and family therapy, crisis and de‑escalation support, and peer support.

Responsibilities include.

• Carry a caseload of approximately ten families.
• Work as part of a collaborative team to support families enrolled in IIBHT.
• Meet each family within 72 hours of intake to identify caregiver support needs.
• Provide phone and email support throughout the week.
• Schedule and meet with adult family members at least once per week at a convenient community location.
• Structure sessions based on caregiver needs and identified goals, such as leveraging strengths, normalizing help‑seeking behavior, de‑stigmatizing mental health challenges, increasing youth self‑advocacy and self‑efficacy, and expanding supports across life domains.
• Maintain high communication with teammates and consult with the clinical team on clinical needs or high‑risk safety concerns, providing unified support to the family.
• Discharge youth once program outcomes have been achieved.
• Attend three weekly meetings (individual, team, and clinical consultation) for professional development and quarterly booster trainings to enhance clinical skills.
• Travel up to 60–80 miles to meet families in their homes and communities (additional travel may be required).
• Complete accurate and timely documentation in an electronic medical record system (EMR).
• Perform other duties as assigned.

This position is part‑time and fluctuates between 15 to 29 hours per week. The schedule is flexible and non‑traditional, based around the availability of the youth and families served. Applicants must possess a current, valid driver’s license, an automobile for work purposes, and proof of auto insurance. Community‑based staff will be reimbursed for applicable mileage.

• High School diploma or equivalent (required).
• Lived experience as a primary caregiver of a youth with complex needs (required).
• Experience successfully navigating child mental health and/or child welfare systems (required).
• Must have or be eligible for Peer Support Specialist credentialing (required).
• Strong organizational skills and attention to detail.
• Excellent written, verbal, and oral skills.
• Ability to manage multiple priorities simultaneously.
• Basic computer knowledge.
• Ability to maintain a flexible schedule.
