Location: Complete Care at Sheridan Commons – Chicago, IL
Shift: Full-Time (Monday – Friday)

We are seeking a dedicated Social Services Director to identify, assess, and address the psychosocial, mental, and emotional needs of our residents while serving as their chief advocate and ensuring their rights are upheld.

Psychosocial Care & Advocacy: Serve as the primary resident advocate and emotional support liaison, upholding resident dignity, self-determination, and rights across all aspects of care.
Assessment & Documentation: Conduct initial Social History and Psychosocial assessments upon admission and at required routine intervals, maintaining strict compliance with state and federal standards.
Interdisciplinary Care Planning: Integrate clinical findings into the Minimum Data Set (MDS) and interdisciplinary care planning processes to deliver holistic, person-centered care.
Discharge Planning: Manage comprehensive discharge plans, organizing community resources, home care, and support services to ensure smooth transitions.
Resident Protection: Proactively monitor for and prevent resident abuse, neglect, or exploitation in strict accordance with mandatory reporting laws and professional licensing standards.
Counseling & Family Support: Provide emotional support, crisis intervention, and adjustment counseling to residents and family members navigating long-term care transitions.
Administrative
Collaboration:
Fulfill operational and clinical duties as assigned by the Facility Administrator.
Education: Bachelor’s Degree in Social Work (BSW) required.
Licensure: Active, unencumbered Social Work license in good standing in the state of Illinois (LSW/LCSW).
Experience: Prior experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting highly preferred.
Skills:
Strong ability to work independently while collaborating seamlessly with an interdisciplinary team.
Proficiency in Electronic Health Record (EHR) systems and MDS documentation.
Exceptional interpersonal skills to build trust and rapport with residents, families, and staff.
