Social Service Director

Job Summary

THRYVE Healthcare is seeking a dedicated and compassionate Social Services Director to lead our Social Services Department and support the social, emotional, and psychological well-being of our residents. The Social Services Director will work closely with residents, families, facility leadership, and the interdisciplinary team to ensure residents receive appropriate support, resources, and advocacy throughout their stay.

• Develop, implement, and oversee social services programs and services to meet the needs of residents and their families.

• Complete and oversee psychosocial assessments and identify residents’ social, emotional, and psychological needs.

• Collaborate with interdisciplinary teams to develop and implement individualized, resident-centered care plans.

• Provide counseling, support, advocacy, and appropriate interventions to residents and families.

• Lead and supervise social services staff and provide guidance, training, and support.

• Assist residents and families with adjustment to the facility and changes in health or independence.

• Coordinate referrals to community resources and outside agencies as appropriate.

• Support discharge planning and help ensure safe and appropriate transitions of care.

• Develop and maintain relationships with community organizations and service providers.

• Ensure accurate and timely documentation of social services activities and resident progress.

• Monitor department performance and identify opportunities for improvement.

• Ensure compliance with applicable federal and state regulations and facility policies.

• Participate in care plan meetings, interdisciplinary team meetings, surveys, and quality assurance activities.

• Maintain confidentiality of resident information and protect residents’ rights.

• Perform other duties as assigned.

• Bachelor’s degree in Social Work or a related field required.

• Minimum 2 years of experience in social work or case management within a skilled nursing facility required.

• Previous supervisory or leadership experience preferred.

• Strong knowledge of social work principles, long-term care, discharge planning, and community resources.

• Excellent communication, interpersonal, organizational, and problem-solving skills.

• Ability to work effectively with residents, families, staff, and interdisciplinary team members.

• Compassionate, professional, and resident-focused approach.

• Ability to maintain confidentiality and handle sensitive information appropriately.
